Instructions for Recertification
- All documents must be PDF’s.
- Do not upload jpegs or Microsoft Word documents.
- Do not upload zip files.
- The maximum document size is 8MB. If your document is more than 8MB, please decrease its size before uploading.
- The Declaration of Minority Status Affidavit is a 2-page document. The affidavit must be signed and notarized. Both pages of the affidavit must be uploaded.
- Please provide your complete 2019 Federal Tax Return (including all schedules K-1s / C / E / G (if applicable). Do not upload your state or local tax returns.
- Do not upload any tax forms that are password-protected.
- Please remove the password before uploading.
- Make sure you check the box to acknowledge that you have completed your recertification.
- Make sure you save the page where you uploaded the documents and acknowledged its completion.
- Processing your recertification can take up to 15-20 business days or more, depending on the number of recertifications due.
- Please use Google Chrome for the recertification process
- You must disable your pop-up blockers
- After you upload your documents, click the save button
